How to Find Online Registration Form

You must bring a signed “registration form” for each player. This form has the waiver and disclaimer information at the bottom of the page.

The registration system doesn’t make this obvious, but if you need to retrieve and print your form(s), follow these steps:

1. Go to the registration site: https://sanjosewestnjb.d4sportsreg.com/
2. Login under Existing Members
3. Under Sub-Accounts, click on your child’s name
4. On the left hand side, click the Forms button
5. Next to the program called WSJ NJB Winter League 2011, click the Registration Form link
6. Print the registration page (will be in Adobe Acrobat PDF format)
7. Repeat steps 3-6 for additional siblings
